Is your graduate program located in the College of Education, Health and Society? (this is important for routing of this petition)
What are you requesting?
Extend time to complete a masters degree (beyond 5 years).
Retain assistantship while on academic probation.
Extend time to complete a PhD degree (beyond 10 years)
Hold employment in addition to assistantship.
Change course credit from undergraduate (400) to graduate (500) - must be submitted prior to grade assigned.
Retain an assistantship with fewer than 9 credit hours.
Change course registration due to incorrect or unintended enrollment.
Enroll in more than 18 credit hours during fall or spring semester.
Change grade mode (e.g., letter grade to credit/no credit)
Enroll in more than 6 hours in winter term or a single summer term.
Withdraw from a course after withdrawal deadline.
Remain in program with low GPA (hold dismissal in abeyance).
